What are anal fissures?

Diagnosis

The diagnosis of anal fissure includes visual examination of the anal area and a digital rectal examination. A digital rectal exam is done by inserting an endoscope into the rectum to easily visualize the tear.

Anal Fissure Treatment

Medical therapy

Initial therapy for an anal fissure treatment is the medical therapy and over 80% of acute anal fissures are resolved without further therapy. Anal fissure treatment goal is to ease constipation and to break the cycle of hard and stiff bowel movement, associated pain and worsening of constipation. The softer bowel movements are easier and less painful for the patient to pass.

First-line medical therapy consists of therapy with stool-swelling agents, such as fiber integration and stool softeners. Laxatives are used as needed to maintain intestinal movements regularly.

Relapse rates range from 30% to 70% if the high-fiber diet is stopped after healing of the fissure. This range can be reduced to 15-20% if patients continue to follow a high-fiber diet.

Second-line medical therapy consists of an intra-anal application of 0.4% nitroglycerin (NTG, also called glycerol trinitrate) directly on the internal sphincter.

Botulinum toxin (eg, OnabotulinumtoxinA [BOTOX®]) has been used in anal fissure treatment (both acute and chronic). BOTOX is injected directly into the internal anal sphincter, effectively performing a chemical sphincterotomy. The effect lasts about 3 months until the nerve endings are regenerated. This 3-month period may allow healing of acute (and sometimes chronic) fissures and resolution of symptoms.

Surgical Therapy

Surgical therapy is mostly recommended for acute anal fissures which remain symptomatic after 3-4 weeks of medical therapy and for chronic anal fissures.

Sphincter dilatation

This procedure is a controlled anal stretch or dilatation under general anesthesia, which is performed since one of the causative factors of anal fissure is considered a narrow internal anal sphincter. Stretching the narrow sphincter helps to correct the abnormality, thus allowing the fissure to heal.

Lateral internal sphincterotomy

Presently, a lateral internal sphincterotomy is the surgical procedure of choice for anal fissure treatment. The procedure is performed on the patient under the influence of general or spinal anesthesia. (Local anesthesia can also be used in the cooperative patient, although it is not always recommended). The aim of this anal fissure treatment is to cut the hypertrophic internal sphincter, thus releasing tension and allowing anal fissure to heal.

Surgical therapy for anal fissure treatment is an option only when the fissure fails to heal or reappear. The usual operation consists of making a small cut in the muscle around the anus (internal sphincterotomy). This permanently reduces the pressure around the anus and allows the fissure to heal. This minor surgery is usually a one-day surgery (you won’t have to stay in a hospital overnight). The success rate is very high for surgical therapy. At least 9 out of 10 cases have healed.
